sample petition to unseal adoption record

Hi there, I am jsut now starting my search for my birth mother and I have no idea where to start. I have no info. on any of my birth family. I was born Dec. 16, 1973 at Alachua General Hospital in Gainesville, Florida. Any help anyone can give me is greatly appreciated. Thanks, Charlotte
