Free Step Parent Adoption Form

Texas - My husband and I want to start the process of step parent adoption on my daughter. Bio has been in jail since she was 3mts. when she was 3yrs i introduce her to my husband (boyfriend at the time) and from day 1 she called him daddy and he loved it. We have been together since and married this year. Now she is 9 and desperately asking me to have daddy's last name. Can you please send me the step parent adoption forms for TX? mari13alv@hotmail.comi will also need to do the parental right's termination, since bio been in and out, looks like he will never be out.if anybody can talk me through the process of how to get parental right terminated when bio in jail?
