Information about the State of Louisiana Adoption Registry: The Louisiana Adoption Registry is a registry established by the Department of Social Services to serve adult adoptees over the age of 18 and birth parents of adopted adults. If both the adoptee and one birth parent register a "match" between the adoptee and birth parents may result in a reunion. Those seeking to find adopted siblings may also register. What can it do for me? You may register if your adoption or your child's adoption was finalized in the State of Louisiana. What if I don't want to be found? DSS will protect your privacy if you do not wish to be reunited with your birth parent, sibling or adopted child. How do I get on the registry? Contact the registry at (225) 342-9922 or 1-800-259-2456 and request a Voluntary Registry packet. The packet includes an affidavit which shall be notarized and a counseling form which shall be completed by a licensed Social Worker or a social worker employed by a licensed adoption agency. One hour of counseling at the expense of the registrant concerning reunion issues is required. Both forms are to be returned to the address below along with a check or money order for $25.00 made out to the Louisiana Voluntary Registry. Once registered, a participant remains registered in perpetuity. If you move it is essential that you notify the registry of your new address and phone number. Will DSS help me with a reunion? If a match is made, DSS contacts the adult adoptees' counselor or an adoption agency located near the adult adoptees home, and requests the counselor or agency to arrange the reunion. Will DSS tell me if my parent is deceased? If you are an adopted adult and DSS has the knowledge that your parent is deceased, you will be notified. Where do I write?
