We filed to adopt our fd. Our cw has never done an adoption before. I have notes about applying for the adoption subsidy from a workshop. However, it doesn't give all the details.
I found a 1 page form on the gov website. Is the application just 1 page? Are we supposed to send in any additional information like a statement from the dr?

I thought that the caseworkers did that. Ours was done when or around the time the petition for adoption was filed I believe. I believe that there is a short time frame to file that also but not sure.

That is the CW's job. They have a form they have to fill out and submit to a unit that approves (or not) the applications. It only comes to you to sign a request, and then they take it back again to give another approval.
Really there is nothing you can do, only the CW.
To be honest if your CW has never done an adoption I would call in her supervisor or someone to help.. When filed our petition just like lakin11 said the CW has to do it. Plus u want to know everything is done right or this could turn out a nightmare if one thing is left out. I would even hire an attorney to help u thru it.
Ohhhh.. I should not have even read this one! I have been upset with the progress of trying to do this for 6 onths myself.. what county are you in Imnop? My case worker in st joe tried this with me.. its not going to adoption team.. we ( the case workers) are handling it.. 6 months later nothing done. and I was asking them these questions and also had everything printed from my own self educating.. I hired an attorney 2 weeks ago.. and I wish I had done it 6 months ago! They kept telling me they were working on it.. these things take time.. and when I needed to do something they would let me know.. then I found out that NOTHING had been done. 2 weeks after hiring a lawyer several things I asked about doing.. have been done and are signed and in the court clerks office now.. it sort lit a fire under them that is for sure!!! I would highly advise.. just get a lawyer.. many on this board told me the same thing.. and I thougth we would be fine.. not so. Now I have joined that team! get one.. they will set the clock in motion. especially if you are in st joe county.
