I can't believe what we are going through just to get this done! We live in southern CA and our insurance is through Kaiser so we've been trying to have a Dr. within our network sign the physical forms and provide a copy of their license for us to have notarized and apostilled. Nobody is willing to do it for us! I finally had a Dr. fill out the form with the notary but then refused to provide a copy of his license! We called Kaiser and they say it is up to the doctor and they will not give us a copy either.
We've called Dr.s in our area out of network and offered to pay out of pocket and they all say we need to see our primary physician - who refuses to help us! ARGH!!
I read other posts like this where people requested stuff from medical board. I just called CA's medical board and even they don't have copies of the licenses! If we MAIL a letter and a $10 check they will send us a certification letter saying the doctor is licensed. Ah!! ARGH!!!
Kaiser's ombudsman or someone in senior management should be contacted. I'm sure that there is someone who can assist you.

Well I am going to write the medical board requesting license certifications and hope Ukraine will be okay with them - have our team review them first, of course.

Ya I already called them and the only thing to do is request a LETTER from them that will certify that he is licensed in CA and will give the license number, expiration, etc... I just hope this is good enough :(
Start crying in their office that all you need is a photocopy of the license they have on the wall to become a mom and you can't believe they won't help you........ (I did this both in the police station since we needed a local person to write a statement that we had no issues in the city and then at the doctors.)
Will work like a charm and the receptionist will get a copy for you just to get you out of the office. Then you just write on it, "I certify this is a true copy" and get your signature notarized and apostilled.

crying worked for me at my local bank-which was a corporate conglomerate-the local person had a heart, but the police station did not. it was my last step and i needed on letterhead that i wasn't a criminal. they would not do it. just some non descript print out. i had to get creative with another way-think i did another state bureau clearance. by the time the second adoption came around they developed a plan with letterhead.
can't believe they won't help.
