I have been married to my wife for 5 years next month and she has a daughter who just turned 10. She was not married and moved away from the biological father when her daughter was just a few months old. Since then he has seen her once and has lost all contact with her. He has not supported her in any way financially either as is seen by the thousands he is behind in child support. I am looking for any help in finding the forms to file to terminate his parental rights and adopt her as my daughter, which she already considers herself to be as do I. We were successful this summer in getting her last name changed. We do not have any idea where the father lives, last we knew he lived in Colorado. Any assistance somebody can provide would be very helpful. Thank you.
