Need help with Petition for Adoption Form

We live in Harris County Texas,has anybody did their stepparent uncontested adoption without a lawyer? The State of Texas and lawyers say you need to use a layer.I got the petition for adoption from the law library. Not sure what parts I can leave out.Does anyone have their petition they used.The lady at the law office said it has to be laid out the same way.including the lines.a lot of it is not applicable.Can you send it to my email momsamuelson@hotmail.comThanks
