Hong Kong is one of the most distinctive international adoption programs available to U.S. families. It is small. It serves a specific population of children. And it has features that set it apart from nearly every other program in the world.

Children adopted from Hong Kong have identified medical or developmental needs. Every one of them. The program exists specifically for children who have not found families through Hong Kong’s strong domestic adoption system. These are children with conditions ranging from mild developmental delays to more complex needs like Down syndrome, cerebral palsy, or significant medical histories.

What makes Hong Kong unique is what families receive in return. The medical and developmental documentation for each child is among the most thorough of any international program. Children are cared for in foster families or small group homes while they wait. In-country travel is approximately one week. And the cost is generally lower than most international programs.

Keep in mind every adoption agency has a different process and every family is different. For families who are specifically called to adopt a child with identified needs and want a structured, well-documented, Hague-governed process, Hong Kong is worth serious consideration.

Hong Kong is officially known as the Hong Kong Special Administrative Region of the People’s Republic of China (HKSAR). It has its own legal system, its own adoption framework, and its own central adoption authority. Hong Kong’s adoption program operates entirely independently from mainland China’s program.

Hong Kong ratified the Hague Convention on Protection of Children and Co-operation in Respect of Intercountry Adoption. The program is governed by Hong Kong’s Adoption Ordinance, overseen by the Adoption Unit of the Social Welfare Department, and uses only approved local social welfare organizations to facilitate intercountry placements. The two organizations currently approved to work with U.S. adoption service providers are Po Leung Kuk (PLK) and International Social Services Hong Kong (ISS-HK).

Hong Kong’s domestic adoption program is strong. Healthy children and those with minor needs are routinely adopted by families within Hong Kong. Children who come forward for international adoption are those for whom a suitable local home has not been found. This means every child in the international program has been genuinely assessed as needing an international family.

All children in Hong Kong’s international adoption program have identified special placement needs. This is not a program for families seeking a healthy infant or toddler.

Children range in age from approximately 1 to 15 years old. Their needs vary. Some have moderate conditions such as developmental delays, speech delays, learning differences, or histories of birth parent mental health or substance use. Others have more significant diagnoses such as Down syndrome, autism, cerebral palsy, epilepsy, hearing impairment, or complex medical conditions like thalassemia.

One of the defining strengths of this program is the quality of information families receive. Medical records are comprehensive. They typically include reports from the time the child entered care, educational progress reports, social history, and as much birth family information as has been recorded. This level of documentation is rare in international adoption and gives families a genuinely informed basis for their decision.

Children wait for families in foster homes or small group homes, not large institutions. They receive good medical care and early intervention services. They have individual social work case managers. This is not a program where children are waiting without support. They are known and cared for while families are found.

Eligibility requirements include:

  • Age: Both prospective adoptive parents must be at least 25 years old. Neither parent may be more than 45 years older than the child
  • Marital status: Married couples are the primary eligible family structure. Single individuals may also apply. Single applicants must demonstrate sufficient childcare experience and a strong support network. Single parents are required to have a companion when traveling to Hong Kong
  • Same-sex couples: Hong Kong’s program is administered by approved social welfare organizations. Confirm eligibility for same-sex couples directly with your adoption service provider, as practice may vary
  • Length of marriage: Couples must have been married for at least 3 years. If either spouse has been married previously, the current marriage must be at least 5 years old
  • Previous marriages: No more than two divorces per spouse
  • Children in the home: The youngest child in the home must be at least 1 year old at the time of application. There is no strict limit on the number of children in the home, though larger families will be carefully assessed
  • Health: Applicants must be in good physical and mental health. Hong Kong is open to applicants with well-managed conditions such as anxiety or depression, provided a positive letter from a treating physician is submitted. Medical letters and psychological evaluations are required for any identified conditions
  • Education: A high school level of education is expected
  • Income: Families must demonstrate financial stability and sufficient income to provide for an additional child. Tax returns and a financial questionnaire are required for families residing outside Hong Kong
  • State licensing: Not all U.S. adoption agencies are authorized to work in every state for the Hong Kong program. Confirm your state is currently served before applying

Travel Requirements

Hong Kong requires one in-country trip. The stay is approximately 7 days. This is one of the shortest in-country requirements of any international adoption program.

Both parents in a married couple are required to travel to Hong Kong to accept placement of the child. Single parents must travel with a companion.

While in Hong Kong, families receive a guardianship order from the Hong Kong court. They also receive the child’s Hong Kong passport and U.S. immigrant visa. Because these documents are prepared before travel, the week in Hong Kong is focused primarily on bonding with the child rather than running between government offices.

One critical distinction: Children from Hong Kong enter the United States on an IH-4 visa, not an IH-3. This means the adoption is not finalized in Hong Kong. Instead, families receive legal guardianship in Hong Kong. The full adoption is completed in the family’s home state in the U.S., after post-placement visits and reports have been completed and approved by Hong Kong’s Social Welfare Department. This is an important difference from programs where the adoption is fully finalized before homecoming. Families should work with an adoption attorney in their state to complete the U.S. finalization process.

Hong Kong’s process is structured and relatively streamlined compared to many international programs. Below is a high-level overview. Your accredited adoption service provider will give you a detailed roadmap tailored to your family.

Step 1: Choose a Hague-Accredited Adoption Service Provider

Select a U.S. adoption service provider (ASP) that is Hague-accredited and works directly with either Po Leung Kuk (PLK) or International Social Services Hong Kong (ISS-HK). Your ASP manages your home study, dossier, matching process, in-country logistics, post-placement reporting, and U.S. finalization support.

Step 2: Review the Waiting Child List and Identify a Child

Hong Kong’s program is child-focused from the start. Many families identify a specific child from the waiting child photolisting before completing their dossier. Your ASP can provide access to the current list of waiting children. A valid or nearly complete home study is typically required before reviewing profiles.

Step 3: Complete Your Home Study

Your home study is a thorough assessment of your family’s readiness to adopt a child with identified needs. Because every child in this program has special placement needs, the home study should specifically address your family’s experience with, openness to, and capacity to support children with medical or developmental conditions.

Step 4: Apply for USCIS Approval (Form I-800A)

Because Hong Kong is a Hague Convention jurisdiction, you will file Form I-800A with U.S. Citizenship and Immigration Services (USCIS) to be approved as suitable and eligible to adopt.

Step 5: Assemble and Submit Your Dossier

Hong Kong’s dossier is notable for being smaller and simpler than most international programs. Documents require notarization but typically do not need apostille or full authentication. Your ASP will provide a precise checklist.

Step 6: Receive and Review a Referral

Once your dossier is submitted and reviewed, the Central Authority will work through PLK or ISS-HK to match you with a child. If you pre-identified a child from the waiting list, you will receive their full referral upon dossier submission. If waiting for a match, the referral process typically takes 6 to 10 months.

The referral packet for a Hong Kong child is thorough. Review it carefully with your ASP and with an independent international adoption pediatrician. Take time to understand the child’s medical history, developmental progress, and any ongoing needs before accepting.

Step 7: File Form I-800 with USCIS

After accepting the referral, you will file Form I-800 with USCIS. This is the petition for provisional approval for this specific child to immigrate to the United States as a Convention adoptee.

Step 8: Hong Kong Legal Process

After the family accepts the match, the legal procedures in Hong Kong take approximately 8 weeks. The Hong Kong court issues the guardianship order. PLK or ISS-HK then prepares the child’s Hong Kong passport and U.S. immigrant visa. Because the visa is typically processed before travel, your week in Hong Kong focuses on meeting and bonding with your child and completing the guardianship process.

Step 9: Travel to Hong Kong

Travel to Hong Kong for approximately 7 days. Both parents must be present. You will receive the guardianship order and your child’s travel documents. Your child comes home with you on an IH-4 visa as a lawful permanent resident.

Step 10: Post-Placement Reports and U.S. Finalization

After homecoming, post-placement reports are required. Reports are typically due at 1 month, 3 months, 6 months, 9 months, and 12 months after your child arrives home. These reports must be completed and approved by Hong Kong’s Social Welfare Department before your ASP can issue consent to finalize the adoption in the United States.

Once reports are approved, you will finalize the adoption in your local family court. After finalization, your child will receive a U.S. adoption decree, a state-issued birth certificate, and you will file Form N-600 with USCIS to obtain their Certificate of Citizenship.

How Long Does the Process Take?

From application to homecoming, Hong Kong adoptions typically take approxi mately 12 to 24 months. This is faster than many other international programs.

Families who pre-identify a child from the waiting list and submit a strong dossier quickly tend to move through the process on the shorter end of that range. The legal process in Hong Kong after match acceptance takes approximately 8 weeks. The in-country trip is approximately one week.

Note that the full legal adoption is not complete until after U.S. finalization, which occurs after post-placement reports are approved. The total timeline from homecoming to full legal adoption in the U.S. is typically several months beyond the child’s arrival.
