Stepparent adoption forms needed for SC

My daughter's bio-dad wants my husband to adopt my daughter...which is great! Now, all I need is the forms for South Carolina! I've searched high and low and I can not find them. I certainly can't afford an attorney! Thanks for any input you can give!
