Step-Parent Adoption. Need Relinquishment Form.

Is there anyone that can point me to the relinquishment form I can send to the father? He has agreed to sign over rights so my husband can adopt my 2 boys. I cannot find it anywhere. Thank you in advance. If you can, my email is cassbaldovinos @ gmail . com

We had a notary do our step-parent adoption when my daughter was 3 and my current husband adopted her (she's now 19). We did it thru a legal clinic locally and they did all the paperwork. You'd need someone to notarize it I would think. I'd ask an attorney so it's all legal.
