I did not name my baby at the hospital; boyfriend and I chose not to (we had a name we would have used) because he was adopted straight from the hospital and neither of us saw him (our choices). A couple weeks later, the Health Department sent me a copy of his birth certificate, demanding I provide a name for him (the hospital had said I did not), sign and have it notarized. There were mistakes on the certificate, my address was wrong, and the birth father's name was listed as XXXXXXXXXXXXXX (not listed). My boyfriend and I both wanted his name on the certificate, and when I returned it to the Health Dept, I included it as a change on the certificate and noted it in a letter I sent with it. My understanding is, because Tyler was adopted, the original certificate is sealed? Is there anyway I can find out if, in fact, they added my bf's name, as requested? We both felt quite strongly about his name being there, so if our son ever decided to search or whatever, my bf's name was there as well (althugh we have a fairly open adoption at this point, so a true search may never be necessary.)

Hi, I saw your letter and was hoping you could help me with a question that has been bothering my wife and myself. When a child is adopted, is the birth certificate changed with the adopted parents name on it? I was adopted and have my adopted parents on my birth certificate. Thanks for any help you can give.
