We have an adopted daughter that is 11 now. we have a problem with school because she was adopted through a family member. Her name was not changed. so the birth certificate was never changed. the school is always wanting to know how come she has two sets of parents. so now we have to show adoption papers everytime for everything even though they have her birth cert. and they know us. what can we do about this.

I don't know the state to state differences, but I would find out how to obtain an amended birth certificate. A name change isn't necessary to get an amended birth certificate. But I would change the name also. Keep the old one, but add yours as well. My kids all felt much better when we were all the same name! Love, Debi
