Hello,
I am a white female, born in 1964 on Long Island. I am sure my birth certificate could be altered, since it contains the names of my adoptive parents.
From what I gather, mine was a private adoption.
I do not have much info.

You need to register your birth certiicate with the NY dept of health dept where you where born call them and they will help you with the form to be filled out. Also the last 5 numbers of your birth certificate can help you find your birth parents at a library in the genealogy section. Just tell them at the library your doing a research if you need them to help.

Are the last 5 digits of your birth certificate number 13554. What you need to do is ask for Rose Ann at 518-474-9600 . She works for NY State Dept of Health Adoption Registry and she can help you register so you can connect with you Bio Fam if they registered. I am registered so if we match they will contact us.

Click on the link bottom of page that reads adoption registry form. Fill out as much as you can especially your aoptive parents name.

Google vital Records in NY and order a birth cerificate and go to [url]www.nyc.gov[/url] and on the right side click on birth certificates (order online now) by using the birth certificate number. Also contact the NY department of Health post adoptions and talk to [url=http://www.health.state.ny.us]New York State Department of Health[/url] and to the right click on forms and then go to adoption section and click on adoptee registration. Fill out form with as much informaton you have and go to bank and notorize it and make a copy and mail it to the address stated on form. They will contact you if they have a match of your bio family that also registered. Your birth certificate number is very important to list.
