A year ago, we changed her name to my last name (he is and was out of state and unreachable). He has had no contact, nor does he want contact. I want to adopt her, but what do I need to fill out, and what do we need to send him to sign at a notery public. He is 100% willing to sign anything. I just need to know what. Please help!!!!

Hi!! If you live in CA, go to your local Juvenile court to obtain forms, including the consent form that the non-custodial parent would need to sign. The filing fee in San Diego was only $20. Once it has been filed, the court will probably direct you to your local health and human services dept where you will be assigned to a social worker. Keep in mind, though, that this could be a 6 month or longer process. But it's worth it. The cost for the social worker's report is about $270 . . . Worth the money, though. And it's a lot less expensive than hiring a lawyer. Since you have the NCP's consent, it should be "smooth sailing". It's more of a waiting period than anything else. Don't let the idea of the social worker scare you. They are all for the best interest of the child. And, clearly, you have her best interests at heart!!! He/she will make a report for a judge to review, you will then have a final hearing and that is it! :0) Good luck on completing your family. We, too, are in the process of my husband adopting my son.
