In the "conclusion space" of the new medical form, the doctor on our medical form wrote "healthy female" for me and "healthy male" for my husband. In answer to every other question, she wrote "healthy". Is that enough, or should she have said we "will live a normal life span and have no mental health problems" (she's not a psych). She is leaving soon for the summer, so if I need a new medical form I have to get it next week. Please answer!Thank you.

KathyI don't know how rigid these have to be, but our agency gave us a dummy form they had filled out and I did exactly what they said. They are usually spot on when it comes to form filling.Anyway, on ours, they wrote "negative" and the date for each section. On sypohilis & HIV, they wrote non-reactive, the lab test number and the date. In the conclusion section, they wrote "found to be in good health. Recommended for adoption. Mentally and physically capable of being a parent. Hope this helps Jane

I would definitely have this redone. "healthy" does not state whether you are positive or negative for any of the Dx on the form. Also, the conclusion needs to be more like what Jane has quoted above. I doubt your current form will even make it past your facilitator. If in doubt, scan it and email it to your facilitator in Ukraine and he will tell you whether he has gotten one approved worded similarly.
