What forms are you looking for? All of the forms you need should be at the courthouse or online. There are three adoption forms. If you need further information on what forms to use and how to proceed I can send them to you. I have them all on my computer. I actually need the forms freeing the child from parental control and the adoption petition. Also the abandoment petition...I can get the general adoption forms online but not those.

I hope the forms I sent you help. You really should get the book "How to adopt your Stepchild in California" That was like my bible for over a year. and I managed to do the entire adoption myself.
