I am an adoptee who has had/has severe health issues. I would have had different care if my health history had been known. I came across this article/handbook and thought I would share the link.

Note that the last page shows who supports this and includes the US Department of Health and Human Services.
Kind regards,

Dickons,
I just came across this post. Thanks so much for the link...I've been wondering how to put together a family medical history, and this is a great resource.
Well done, Dickons...thanks again!
Best wishes,
Susan

Thank you so much Dickons. I just earmarked this link in my 'favorites'. As it happens, both my parents are heart patients. In fact, in about 5 hours my dad is having a cardiac ablation done. He had a valve replaced 2 years ago. My mom had 4 stents put in 12 years ago and a quad bypass 11 years ago. There is diabetes and cancer on both sides of my family.
I am going to do one of these books and make copies for each of my siblings. I also plan to make one for my soon-to-be adopted son with info about his first-family.
