Hey gals! I hope you can help me out ;) I finally got someone to send me a copy of the new per diem form, arg.. why is the form only in excel online?? I can't open it.. blah!
Ok anyway filling it out , I have two quick questions..
What did you put for the service code? Is it the same as billing code was on the old form or did you leave it blank?
What did you put for the Referral ID?? The instructions say it's a 15digit # but I have no 15digit number.. lol.. I was thinking maybe the Case #??

I know! I can open it, but cannot print it. Luckily I had the letter they sent in the mail and made a copy of that one. I e-mailed the lady back and forth this week to get it figured out. She said foster parents do not need a referral ID # and to leave it blank. She gave me a service code of 20721. I am not sure if this is a code unique to each person or if all foster parents use the same one. I also asked about the person ID #'s. The form says they are 8 digits. My kids' are longer. She said that is fine and not all are 8 digits. Hope this helps! I am heading to mail mine first thing in the morning and hoping for a quick turn around...

The st # is on your warrant summary where it says vendor id ST000000. I have the paper they sent to us with directions on the back and this is what it says the number is, so everyone should be different. Also fyi, you are suppose to put the county that placed the kids in your home. It says the service code is not required if a referral id is entered unless you have the unique 5 digit number for the type of service that providers offer to dcs. Hope this helps.
Btw, when this gets to Indy does it not have to be sent to someone at the local office to verify the kids are still in care or how is that working now?? Like before when the caseworker had to check off on them before they went to state, are we bypassing that now or what?? I'm so confused with all this change::coffee:
