I did the Placement Process training at Marion Co. DCS today. There were a few changes in filling out the claim form.

Look for Standard Invoice.
#4 Invoice Number
For the coming pay, this should read July 2010.
#5 Invoice Date
This should be the same date as #23
#14 Service Code
This number is 20721. This number is the same for everyone.

Sign only in BLUE ink.
DO NOT SEND IN PHOTOCOPIES ONLY ORIGINALS.
There is only a 10 day window to get these in. For example, if you sign and date it on Aug 1st and don't bother to mail it until the 15th, they will reject it. You still have 60 days to file.
P.S. The training was very good in case you ever get a chance to take it.

how did you even find out about the training? i didn't get any kind of notice....is there a way we can sign up for a training date in the future?:confused:

It was on a separate sheet of paper in the last Nesting Notes. The training wasn't specifically for the claim form. It was included as part of the placement process. The training was presented by all the licensing workers.
I went ahead and filled out my form last night before I forgot what I was supposed to do. . Do you know how long it took me to find a blue ink pen? There were two in my whole house and one of those was dried up.
The form can be saved now so at least I won't have to dig out forms from previous months.
Thanks Becki!! Ugh @ them always changing something.. I swear they are purposely trying to screw us up! ha.. I cannot wait to have my adoptions finalized so I dont have to deal with the anymore.. Then I'll just be ranting if my subsidy isn't in on time.. ha
No. I can't Imagine that KidTraks would even consider if they are making such a big deal about originals and blue ink, but who knows. It sure would save them sometime.

Ugh, it's still in excel format.. Am I the only person on earth that doesn't have excel on their computer?? ugh!
funny.. no one is aware of these changes unless someone here relays the info. I already send July in.. and did it with the same codes and numbers I did last month! glad it was my last one!! (sort of!)
Actually, Paula Johnson who is a licensing worker in Marion Co. Called up the Auditor's Office the morning of the training so she would have the most up-to-date info. It will probably change again and soon.
