Help w/ Forms

My husband is adopting my children from a previous relationship. The father has had no contact with the children and has failed to support them in a year (he has actually never supported them since we seperated in 2007). He was recently incarserated for meth. He is unwilling to sign his rights. I have forms 200, 210, and 215 filled out. My question is that it asks for me to attach "Petition to end parental rights". I was told that it isn't an actual form. Is there a standard format for this? I am from California. Thanks!
