Help! I need the forms for a step parent adoption in TX

I am in desperate need of the forms for a step parent adoption in TX. I have seen some posts on here that has the forms, but I am unable to download them. If anyone has them, can you please post them here and/or send them as an attachment in an email?

My ex gave up rights in the divorce so do i just include a copy with my paperwork instead of filling out the first two forms? thank you so much for all your info. it is a great help.

ALSO it is for 2 children do i put both names on each form or fill out two forms for each?
