Adoption Forms

:confused: How do we get adoption papers with out an attorney here in Indiana? We dont need an attorney for this adoption because the bio-father will sign all papers, because he hasnt been in my daughters life for 15 years.so there is no fighting in this adoption. So can anyone tell me how to get started or where to get the paper work.I have tried the Library, clerks office and they said try department od social and health services, so I did that and they said they dont carry those forms. So I am stuck on what to do. My husband has raised my daughter for 12 going on 13 years and she wants this adoption to happen, and so do we. Thank you wellington34@hotmail.com
