The Dr. wants to put R on meds. He's 4 yrs old, but is very very small 35 lbs and 39.5" We're very weary of putting him on meds and really only feel he needs it when we break from the routine. He also has FAS & SID. I'd love to hear anyone's experiences with this.

As a foster child, you could get in trouble for refusing a Dr. suggestion. However, I would make him clearly clarify why he wants to use them, if change in diet will occur or could help without meds, non med alternatives etc. Also, journal and see if meds make a difference.
My son was on tons of meds at 4 and didn't need any of them. He needed room and time to run and play, time to attach to me and the work to aid the attachment. In my opinion, unless it's medically necessary to sustain life or remove pain or stop violence, a child that young does not need meds.

Thanks for the info. We're very close to finalization so don't need to worry about him being a foster child. R's caseworker is supportive of us no matter what we decide. The only thing that seems to work for R is a lot of structure and not breaking from the routine. Whe we break the routine he can't control his actions, he's very clumsy and will cosnstantly run into things, and end up hurting himself (but he doesn't even notice the boo boos).

That's likely due to the FAS. My child with FAE just can't funtion if there is a break in his normal schedule. I don't think a pill will fix that. When he starts school, it might be worth a try with the ADHD drugs to see if they help him focus better, but I doubt they will have any affect from what you describe. If it were me, I'd let it go. Like you said, if life is structured, he has no problem.
