I'd encourage you to check with your agency about this, but, for us, adopting from Ethiopia, we filled out the Embassy in Addis Ababa, Ethiopia. Where's the US Embassy in Guatemala? That might be where you fill this out, if you go to the embassy when you are there to complete your paperwork.
