At the time of the birth, the birth parents (who are the legal parents at birth) are named on the certificate.
Once the adoption is finalized (it can take a few weeks to a year, depending on the state/county) the original birth certificate is amended to reflect the new legal parents (adoptive parents) ad the original certificate naming the birth parents is usually sealed.
Thats about it.

So technically, we will not even have a birth certificate at all until it has our names on it?? Does the court automatically make a new birth certificate with our names on it and send it to us or do we have to have it done?? Basically, I want to know if it is our responsibility to get it done or is it the courts? Do we do anything to ensure the change? Thanks!!
It is your attorney who does all of that - it is a form you fill out that is sent at the end of the process.
You may have a copy of the original certificate - but no, you won't have one until its all done :)
As Brandy said, we filled out a form at finalization.
Because our arrangement is not fully open, the original was sealed by the court and resides in the county where our son was born. We have the copies of the new one issued which names us as parents.
We were given an original from the state where our daughter was born by our lawyer. It has the bmoms info on it. After finalization we filled out a form at the court house and it was sent off to the state she was born in and an amended one was issued with our names as parents.

We were curious about how that works. Neither one of us knew. We just figured that we got custody papers and that would be it. Thanks for answering our question!!!
There will be an original birth certificate with the first mothers name and information on it that will be kept in the babies adoption file then the baby will have an ammended birth certificate with the name you give the baby along with your name and information on it. At least that is how it was done with my OBC and papers.
