Petition to terminate rights

Does anyone have a sample to use? I'm in San Mateo county and we already filed the adoption petition but now have to terminate BFs rights I guess before we can move forward with the adoption. He hasn't had any contact in years and we don't actually know where he is so we'll have to do notice by publication but I'm working on the termination petition if anyone has an example so I can get an idea about wording, I would really appreciate it. Thanks!
