Post Adoption Agreement

We are getting ready to do an open adoption, and we are interested in writing a Post Adoption Agreement with our child's Birthmother. But, I want some ideas as to what we should include in this agreement. I would love to here what Birthmother would like to have in a Post Adoption Agreement. Things that work well and things that didn't work so well. Things that you wish you would of included in your Post Adoption Agreement.
