I am trying to find a form to have my ex husband reliquish his rights/ my husband to adopt my children. Anyone who can help I would appreciate it. My ex has not seen his kids in 5 years, does not pay any child support. My new husband is the only father they have ever known.

seems like your attorney should be able to provide this form and will be the one to present it, however, I know that in my state, there is a statute (?) in the law that indicates abandonment after no contact over a certain period of time where it constitutes abandonment and the relinquishment is not necessary if you can prove it.
