Can't find the form...

My husband, daughter and I all want him to adopt her. Her bio-dad has agreed to sign over any rights...However I can not find a Voluntary Termination of Parental Rights form. This I think is the last form we need, and I have looked everywhere on line for it. Help! Does anyone know where I can fined this form for the state of CA?? Thanks so much! Ash
