Searching for parents.

Hello, I don't know anything about my adoption. I'm trying to get answers but my adoptive parents will not tell me anything. I live in Wisconsin and have my adoptive birth certificate from Wisconsin. Awhile back you guys sent me a pdf form where I could request my real birth certificate or my real identity from Wisconsin. If you guys could send me a link to that form again that would be great.
