My name is Jeffrey & I was born at the Appleton Memorial Hospital on 11/12/1969.
I would like to know any health history.
I understand 100% that I may never meet my birth parents, but I would at least love to see pictures of them when they were younger up to today. I've always wanted to know if I look like them. I am 5' 11" tall, 175 lbs, blond hair blue eyes & am 40 years old. Its crazy because I don't even need to know their names but would give anything to know what they look like. It would also be nice to find out if any of my hobbies are simular to theirs.

Hello, I am an adoption search volunteer/advisor.
If you requested the non-identifying information on your birth parents, you would receive info, such as, Race, Nationality, Education, Occupations, Religion, birth grandparent social history, sibling information, hobbies & talents, ages of your birth parents at the time of your birth, and the reason for the adoption. The State of WI, or the Adoption Agency used would provide this info, upon your written request.
You could also sign a Disclosure Affidavit releasing identifying information to your birth family. I can send you the Disclosure form.

I was just wondering if you ever found any history out from the state of Wisconsin. We are looking for a sibling born in 1969 and I am just following up on all potential leads. Let me know if you would like to pursue this further.
