Just before children move in for adoption placement, what kind of questions/info should I ask the caseworker? Ex., copy of IEP, availability of counseling.....Any legal questions?

Here is a list that I was given by someone in another state who was new at it too. Just be warned, that you may not get all of the answers. And all of the questions will not apply to all situations.

- Name, Birthdate, Sex
- Does the child have Siblings? If so, where are they (home other placement) ? Why aren't the siblings together?
- Has the child been in care before?
- Why is the child being placed in care now? (What happened, when, where and by whom?)
- Does the child have any special needs (medical, social, behavioral)? If behavioral, inappropriate or abusive to self; adults; children; property; pets ? Have medical needs been confirmed by physician? Is there medication to be expected upon transition? Child's doctor ?
- Any special cultural or heritage needs or restrictions?
- Does child wet the bed ? Is the child Potty Trained? Is the child on a bottle ? Use a pacifier ?
- Is the child receiving treatment of any kind? What, when, where ? Who will transport child for treatment?
- Has day care or school been arranged ? Again, where, days, times, who transports?
- What is the legal status of the children?
- Will any family members be allowed direct contact with the children ? Who / how /when ?
- What is the visitation plan? (When, who, transportation)
- Social worker Name, Phone and CELL PHONE
- Guardian ad litem
- Has child made allegations of abuse against other foster parents? (When / Situation / Status)
- What "stuff" will the child bring ?
- Has the child ever broken the law ?
- Does the child smoke ?
- Is the child sexually active ?
- Do you have the childes Placement Plan with you? If not when will I get a copy ?
