Hello,
I am applying for a replacement ctizenship document (form N-565) now that I have readopted my daughter and have legally changed her name. I have a few questions because I don't want this sent back to me due to me filling it out wrong.
In Part 1 - it asks for family name and given name. Do I put my daughter's birth name or do I put the name I gave her and my last name?
Also, the form requires the certificate number. Is the certificate number the A number above the word Citizenship? Or is the A number after CIS Registration Number? Or is the number after the INS director's signature?
Part 3 - The form asks: "My last certificate or Declaraion of Intention was issued to me by:" USCIS Office or Name of Court:
I didn't save the envelope the CoC came in, so what do I put down as my answer?
Part 4 - I do put the date of the court decree where her name was changed in the blank line, correct?
Part 7 and 8 - I leave 7 blank since my daughter is 2 and can't sign and I just fill out part 8, correct? And if so, should I or do I put that I'm her mother below my printed name or signature?

Family name is your daughter's legal last name. Given name is her legal first name.

The Certificate number is above the word Citizenship. The number after CIS Registration Number is the A# you need to put in the box next to Certificate number.

All IR3 visa COC's were issued by the Buffalo NY office.

There has been much debate about this. And what has worked for some hasn't for others. It seems to be up to the random whim of the person processing it. Personally I just signed my name in Part 7 and left it at that and had no problems. I would pick either Part 7 or Part 8 and sign and leave it at that. The information on the front makes it obvious she is a minor . No guarantees. This is USCIS you know! But that is what I would do.
