Has anyone had success with the forms Adopt 330 & 331?

I am trying to help my Mom locate her birth sibling, a Brother. We contacted the family law of the superior court in the county he was born in. They could not offer any information and told us only the adoptive family would be able to get that information. The women I spoke to suggested filling out a form called Adopt 330 & 331. She said it was a request for appointment of confidentiality with an intermediary. I'm so new to this and may be this is a common path to take. I'm just curious if others have done this and have found success. I live in California, the forms may be called by a different title in other states. If you've been down this route I would love advice! :)
