Born in Chicago (Cook County) on 8/28/61 to a 17 yr old birth-mother and 21 yr old birth-father at Mercy Hospital. Dr JE Kavanaugh was the attending physician. Adopted through Catholic Charities (although they have no record). I just found out about requesting an original birth certificate from the state, which I will do (as soon as I find out if I am moving to another state).
