Which Mother's Maiden Name do I use?

Hi there, I'm in the process of being adopted as an adult by a very close friend. After I am adopted... When I'm filling out forms, which mothers maiden name do I use? My birth mother's or adopted mother's? I can't find an answer anywhere so I'm hoping this will help... Thanks!
