Filling out forms

I'm curious what other foster parents do when they run into forms that ask for parent/guardian info. Technically we are neither parent or guardian but I think I've filled in my name in such forms before. I think I've also crossed out parent/guardian and put fostermom instead. These forms are typical of doctors offices and schools.
