Dr. Art,
I promised a close friend of mine that I would come to you and ask you this question. They have a 2 year old son who has a genetic family history of a seizure disorder and sickle cell. His behaviors are such that he is in constant motion. He is always running, jumping, they can't get him to sit down, sit still, can't even get him to sit in a seat to eat and can't even control himself enough to use a spoon. They have asked for help from the pediatrician and therapist but they just say, "you are good parents, keep up the good work". That isn't working for them. They have a 3 year old who has FAS and a 4 year old. They are overwhelmed and at the end of their rope. The 2 year old had a cold recently and was prescribed a cold medicine. It calmed him down so much that he was able to stop and read a book, eat with a spoon, and concentrate. THey would like to see him function at that level on a regular basis. They need help and they need to know what kind of specialists to ask to see. WHat sort of specialists would you suggest?

I would start by having the child evaluated by a mental health professional (social worker, psychologist) who has substantial experience evaluating and treatment adopted and foster children who have attachment problems AND who has specific training in this area...it they've not been trained they probably don't know what they are doing (look at [url]www.attach.org[/url] for some names in the area).
Second, I'd suggest getting an evaluation by an Occupational Therapist who is Certified in Sensory-Integration. If they've not been certified in that, don't use 'em. Lots of OT's know about SI, but are not certified.
Third, you may want to consider an evaluation by a developmental pediatrician.
Last, after all the above, consider a neuro-psychological evaluation by a neuro-psychologist...but only if one of the above recommends it to sort out learning and neurological issues.
