Ethical

I am not sure about this but I hope there are some counselors that may have the answers. Can or would a counselor send out a letter on a clients behalf without the client even reviewing it? I have always assumed that the client would review and approve it before it was sent out. Am I incorrect? Thanks for your superb knowledge:)
