Start with filing a Petition For Adoption with the courthouse. I'm doing the same research.

Usually with Form ADOPT-200.
Fill it out, turn it in.
At least in Santa Cruz County.
Then they sent a copy to Social Services who make a report.
When the report is finished, you schedule a court date.
At court, you present ADOPT 210 and ADOPT 215
