I was wondering if there are any places in the world that have evolved their legal certification process to include a separate birth certificate and adoption certificate? This would make more sense as not to hide and lie on legal documents who actually gave birth to a child.
Is this straight adoption? I have an amended birth certificate; there is an original and there is a separate adoption certificate.

So you have your original birth certificate and an adoption certificate so you can see who your birth parents are on your birth certificate and you see your adoptive parents on your adoption certificate? Where are you from?
My children are adopted from Guatemala. We have their origional birth certificates, the adoption decrees that show their birth parents names and our names, as well as their new birth certifcates showing DH and I as parents.
