Stepparent adoption forms for Nevada

My husband of 5 years wants to adopt my 13 year old daughter. We went to our local court house but they don't carry the papers. They gave me a bunch of websites to try and I cannot find them on those either. If anyone knows of a way to find these papers or have some they can email me we would GREATLY appreciate it!!! I just registered for this site, have the same question as you. Were you ever able to find a do-it-yourself adoption form for Nevada? I have read a few other posts from ppl about this and was hoping to get someone from the same state...
