If anyone in Michigan can help me get started with the search process, I'd be so grateful. I've gone on michigan.gov and printed out the "[URL="http://www.michigan.gov/documents/FIA1925_10575_7.pdf"]Request by adult adoptee for identifying information[/URL]" and "[URL="http://www.michigan.gov/documents/DHS-1920_134363_7.pdf"]Release of information authorization adult adoptee[/URL]" forms.
With both forms, "birth mother's name" and "birth father's name" are fields to be filled in. Is this a requirement? If I don't know this and leave them blank, will the dept of human services void my request?
The forms also ask for "name of probate court" and "name of placing agency." As far as I know, I was born in Southfield, Michigan and was placed with Catholic Social Services. Is it enough to put the name of the probate court that's listed for Southfield? And would the Catholic Social Services automatically be the branch in Oakland County (which is where Southfield lies)?
Also, once I do fill out the forms I'm not sure where to send them. I found an [URL="http://www.michigan.gov/documents/FIA-AdoptPub439_12970_7.pdf"]online pamphlet[/URL] that had this address:
