Hi! I just received my first foster placement a few days ago, and I am hoping for some advice on how to complete the Claims forms and submit them. I know that our local Indiana agency uses a new online system which is NOT working for me. :hissy: If someone could please let me know how I submit the claim form, where etc. I would really appreciate it!

My first month I contacted my licensing worker and she made up a dummy copy with all the correct numbers and info on it as a guideline for completing all the others. I was originally so confused because they were asking for numbers that I had no idea what they were even for. She was very helpful. She also told me all claims had to be signed in blue ink and had to be mailed within 10 days from the date you put on the claim. I could not have filled it out properly without her help. My advice is to contact your licensing worker. Good luck!

I am from Indiana and recently learning how to fill out the forms. First you need to get signed up for a kidtraks account online. The phone number where you can call and get help signing up for kidtraks account and help with the forms is 1-877-340-0309. The address to send your completed forms to:

Also you willl receive paperwork on your foster children in the mail that has the information to help fill out the per diam form. Your travel form you'll have to keep track of your mileage and enter it on the travel forms for both ways. Use mapquest along with their shortest distance feature for your mileage which is under options. Both your per diam and travel form should be sent together but on seperate sheets to the address listed above. The paper work can be a headache when your just starting out but don't worry you'll soon get the hang of it :)
Yea kidtraks is pretty useless. I can also only see when my invoices are received and when warrants are posted for payments. Still have to have it though :(

As long as you include the required information you should be fine. I have been doing claims forms for 19 months and just received kidstraks access a month ago. If you wonder what to fill out, there is a PDF file on in.gov/dcs site. just search kidstraks foster parent invoicing and it tells you how to fill it out.
Thanks. I finally got KidsTrack to load but no mention of my invoice submitted. What is the typical turn around? In class they warned us a month but I was curious to see if that is true.
