I'm 29 now and my mother recently passed away and she always talked about a sister I have that she had to give up for adoption. I have done some research and found that I can submit a request to the department of health for her adoption name and info. Its says the request must have notarized identification of relation which I'm assuming intails taking my ID and birth certificate to a notary proving my birth parents are the same as hers , but I was curious if anyone had an example of how this should be written as well as how to be submitted? By mail in person ect. Also if you know of an easier way I'm all ears. Thank you.
