You should have received a decree of adoption and an amended birth certificate. You can go to the court where the adoption was granted and get a new certified copy of the decree. Usually there is a minimal charge for this.

I gave birth to a Son in 1973 and have been searching for years to be reunited with him. I do not want to interupt his life. I just need him to know the truth of his birth. I have a family medical situation I feel he should know about. My son was adopted through Indiana Welfare Program in Porter County Indiana. He was born in Fort Wayne, June 24. If you can help in any way I pray you will reply back.

First I would the welfare dept. who handled the adoption, and ask them to send a letter to the last known address of the adoptive parents.
If that doesn't get a response, then mail a letter to the court in Porter Co., asking the judge to open your file.
I did this (no reply to the letter but it didn't come back to the welfare office, either), then sent a letter to the judge, who ordered the file opened. My husband's (the adoptee) is deceased, and I wanted the information, esp. medical, for our 2 boys.
