DD Form for Insurance coverage?

Hi there,

I recall reading somewhere in this vast amount of information about a DD form we can use to show our agency that our child (children) will be covered by Tricare after they are adopted. Does anyone know the form and number and website where I can get this? I should have written it down when I saw it, but hindsight is 20-20!

We are 8 weeks out before the birth and trying to get this last minute paperwork!

Explains the change in '02 that allows coverage for pre-adoptive wards. DD form is 1172 - application for ID Card/DEERS entry

Are you at Langley AFB in Hampton VA? If so, go to NAB Little Creek in Norfolk to get your child added to DEERS - they seem to be the only DEERS office in the area that understands how to do this. We were stationed at Oceana in Va Beach and they didn't have a clue how to go about it.
