adoption of my 5 year old

I am the singel mother of a 5 year old whose father has never seen her- he left when I was four months pregnant. I really do not want her to have contact with this man until she is at least 18. Here is my question- I have a very close male friend who is homosexual and desperately wants a child. We are looking into him becoming her legal father. We would like for his name to be put on her birth certificate and for her to take his last name. Is this possible and where do I go from here? He is a wonderful man and this would be a great gift to both of them- for him a child and for her a great dad. :)
