I was born in the Salvation Army Home for Unwed Mothers in Sand Springs, Oklahoma. My date of birth is 11/14/70 and I am female. I'm looking for my birth mother or father. I am trying to get a birth certificate for a deceased person,,,Mary Hillison(my mother who was adopted). What does this mean? Do we have to give the information for the real parents(who we don't know anything about or the adoptive parents...? Can someone help me please...Thanks so much!!!!

I got the following letter from the Illinois Department of Public Health (the people I sent the birth certificate request to)

This office has received a request for your birth certificate. We think we have located your birth record but some important information was omitted at the time the certificate of birth was prepared. If you will complete the enclosed form, we will amend your birth record and send you a certified copy.
WHAT DOES THIS MEAN. The form asks for full name of the child, full name of the mother, father, where did parents live at the time this child was born, color or race, parents date of birth and place of birth, parents social security number, occupation at the time this child was born, other children of this mother who were born BEFORE this child was born...

I was born at the Sand Springs Salvation Army Hospital for Unwed Mothers. My birthdate is 5-3-69 and my adoption was arranged by Family and Children's Service in Tulsa, OK. I have joined the Reunion Registry, but have not had any matches in the last nine months. The Department of Human Services cannot locate any information on my adoption and suggested that I conduct a confidential intermediary search. Before I spend the $400 on this search, I would like to try to find more information on the Salvation Army Hospital and the Family and Children's Service to possibly locate hospital and adoption records. Any suggestions?

My name is Lori and tonight I found out that my mother Brooke ****inson had a baby girl in 1970 my aunt told me that her name was stacy or stephanie my mom went to the home for unwed mothers and gave this baby girl up for adoption. My name is David. I just joined this website and I too am adopted and looking for a sister that was born in 1970 at the Salvation Army home for unwed mothers in Sand Springs. My twin brother and I were born June 8, 1969 and found our birth parents in 1999. We also found 1 full brother, 1 half brother, and 2 half sisters from our birth mother in 2009 using information that was given to us and actually finding them on Facebook. Our Grandfather and Aunt told us that there was 1 more out there and she was born in 1970 and put up for adoption, and even possibly adopted from a wealthy family near Riverside in Tulsa. This is not a match but my husband was also born at the Sand Springs home on 11/23/70. He recently located his birth mother and I thought there might be a chance that your mother and his had crossed paths.
If you are still looking, I would be glad to ask her if she has any memories that might be helpful.
