Private Guardianship

I am in Canada and in order to have an older child come in to the country we need to do a Private Guardianship. Canada has laws that specify the age you must be under to adopt. What I need is the wording in a Guradianship document. I am a legal secretary but our law firm does not do this sort of thing and my boss would not be too pleased if he found out I was wanting to get a child. So you can see why I have to find it here. Has anyone had any dealings with this document. Please help me accomplish locating it. thank you!
